 There are many different definitions of organization development available; almost all of them try to define yet another management plan and process, which if believed and followed could result in holistic improvement of the organization.

Most professionals when asked to find flaws or shortcomings in their work often tend to find reasons external to them. Take an example of a typical lesson learnt session of a project, one would hardly listen to any reason that might indicate something that can be improved in inherent way the project is managed or carried out.  The team would point fingers at customers not putting the requirements properly, some other team not fulfilling their dependencies on time and so on.
